Short answer

Pain in ankylosing spondylitis often comes with inflammatory stiffness, commonly worse after rest and eased by appropriate movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How this pain behaves

Ankylosing spondylitis is an inflammatory condition affecting the spine and the joints where the spine meets the pelvis. Pain may be deep in the lower back or buttocks and can alternate sides. Morning stiffness, stiffness after sitting, reduced spinal movement, and discomfort that disturbs sleep can form a useful pattern. Some people also notice pain where tendons attach to bone, such as the heel or ribs. A flare may bring more pain and fatigue than usual. A diagnosis cannot be made from pain alone, so tell your clinician how symptoms change with rest, movement, and time.

Ways to manage symptoms

Follow the treatment plan prescribed for you and keep a record of flares, stiffness, sleep, and activity. Regular, low-impact movement can preserve spinal and hip mobility; Exercise Therapy and Physiotherapy can tailor stretching, posture work, breathing exercises, and strengthening. Hydrotherapy may make movement more comfortable when land exercise is difficult. Anti-Inflammatory Medication may be suitable for some people, but it should be selected with a clinician who knows your health history. Avoid prolonged bed rest, and pace activity during a flare rather than stopping all movement.

When to seek care

Book a review if pain is steadily worsening, your usual treatment no longer helps, or stiffness increasingly limits daily activities. Seek urgent care for a new inability to walk, severe pain after injury, new limb weakness or numbness, loss of bladder or bowel control, or numbness around the saddle area. A painful red eye with light sensitivity or blurred vision also needs prompt medical attention, because inflammation outside the spine can occur with this condition.

Questions for your doctor

Does my pain pattern fit active inflammation or another cause? How should I adjust exercise during a flare? Which medicines are appropriate with my other conditions? Would a physiotherapy review help my posture, breathing, or mobility? What symptoms should trigger an urgent call?

Questions people ask

Can movement help ankylosing spondylitis pain?

Appropriate movement often helps stiffness and mobility, but the intensity should be adapted to your symptoms and treatment plan.

Does every backache mean ankylosing spondylitis?

No. Back pain has many causes. A clinician uses the symptom pattern, examination, and sometimes tests to assess inflammatory conditions.
