Pain during sex penetration: causes and next steps

Burning or stinging as penetration begins points more toward the vulval or vaginal tissues. A feeling of tightness can occur when pelvic-floor muscles contract protectively after pain, stress, or previous injury. Deeper aching may be linked with endometriosis, an ovarian endometrioma, or pelvic inflammatory disease. Genitourinary syndrome of menopause can make vaginal tissue thinner and drier, while lichen sclerosus can cause soreness and skin changes. These conditions can overlap, so a careful history matters.

Do not force penetration. Allow more time for arousal, use a suitable lubricant, and choose a position where you control depth; stop if pain increases. Avoid scented washes around the vulva. Book a gynaecology or primary-care review if pain repeats. The clinician may inspect the skin, assess pelvic muscles, test for infection, and consider imaging for deep or one-sided pain. Treatment is tailored to the finding and may include tissue care, infection treatment, pelvic-floor therapy, or management of an underlying condition.

Seek prompt care for new pain with fever, foul-smelling discharge, sores, bleeding after sex, or worsening pelvic pain. Get urgent help for severe sudden pain, faintness, vomiting, or heavy bleeding. If penetration is impossible because of pain or muscle spasm, arrange an assessment rather than repeatedly trying.
