Pain During Sex Deep in the Pelvis

Deep pain during sex can reflect pressure or irritation involving the cervix, uterus, ovaries, pelvic muscles or nearby tissues. The timing, depth, side and accompanying symptoms help a clinician narrow the possibilities. Pain that is new, persistent or interfering with intimacy deserves an assessment; it is not something you need to tolerate silently.

Pain with deeper penetration may be linked with endometriosis, an ovarian endometrioma, pelvic inflammatory disease, uterine fibroids or tightened pelvic-floor muscles. Endometriosis commonly presents with deep pain during sex and may occur alongside painful periods, pelvic pain or bowel and bladder symptoms. An ovarian endometrioma can make movement or pressure uncomfortable on one side. Genitourinary Syndrome of Menopause can also cause pain through dryness and tissue sensitivity, even when the discomfort feels deeper.

The symptom does not identify one condition by itself. A pelvic examination, pregnancy assessment when relevant, infection testing or ultrasound may be considered according to your history.

Pause or change any activity that causes pain. Try slower movement, more lubrication and positions that let you control depth. Note whether pain is one-sided, linked to your cycle, present after sex or accompanied by bleeding, discharge, fever or urinary symptoms. Bring that pattern to a gynaecology appointment. Treatment depends on the cause and may include pelvic-floor physiotherapy, hormonal treatment, infection treatment or a discussion of procedures such as Laparoscopic Excision of Endometriosis.

Arrange prompt medical care for severe or worsening pelvic pain, fever, faintness, vomiting, unusual bleeding, foul-smelling discharge or pain with a positive pregnancy test. Seek urgent help if pain is sudden and intense, you feel very unwell or you might be pregnant. Otherwise, book an appointment if the pain returns, lasts beyond a brief episode or changes your sexual activity.
