Why position can matter
A position can change penetration depth, angle and pressure on the vaginal wall, cervix or pelvic muscles. Pain near the opening may involve dryness, vulvodynia or lichen sclerosus; deeper pain may occur with endometriosis, an ovarian endometrioma or pelvic inflammatory disease. Muscle guarding can begin after an earlier painful experience and then make penetration hurt more. The position pattern helps a clinician understand the symptom, but it does not identify a diagnosis by itself.
Make sex more comfortable
Stop when pain starts and choose positions where you control depth and pace. Try more lubrication, longer arousal and a gradual approach; communicate a clear stop signal. Note whether pain is at the entrance or deep inside, which movements trigger it, and whether it occurs outside sex. If the pattern persists, a gynaecology or pelvic-floor assessment can look for tissue irritation, infection, muscle tension or a pelvic condition.
When to seek care
Seek urgent care for sudden severe pelvic pain, fainting, fever, vomiting, heavy bleeding, or pain when pregnancy is possible. Book a routine appointment when pain is recurrent, worsening, associated with bleeding or discharge, or limiting intimacy. A new symptom after menopause also merits assessment.
