Pain during sex after episiotomy

Pain during sex after an episiotomy may come from scar sensitivity, incomplete healing, dryness, or pelvic-floor muscle tension. The cut can leave a tender or tight area, and protective muscle tightening may continue after the skin has closed. An examination can check the scar and identify other causes.

Stinging or stretching at the entrance points toward surface tissue, scar sensitivity, or low lubrication. A deeper ache may involve pelvic-floor muscles or another pelvic condition. Note whether the area feels itchy, raw, numb, tight, or painful when touched. Pain is not proof that the episiotomy has reopened, but visible separation, discharge, or increasing soreness needs review.

Do not push through pain or use penetration as a test of healing. Try non-penetrative intimacy while the area settles. If you resume, use lubricant, take time, choose a position that allows you to control depth, and stop if discomfort rises. A clinician can assess scar tissue and pelvic-floor coordination; physiotherapy, guided scar care, or treatment for dryness may be considered after examination. Keep the area clean with water and avoid perfumed products.

Contact a clinician promptly for fever, spreading redness, pus-like or foul-smelling discharge, wound separation, heavy bleeding, or severe pain. Seek review for bleeding after sex, a persistent raw area, new urinary difficulty, or pain that does not gradually improve. Urgent assessment is appropriate if severe pelvic pain occurs with faintness or marked weakness.

Ask whether the scar is fully healed, whether its position explains the pain, and whether pelvic-floor physiotherapy or scar treatment could help. Tell the clinician when the episiotomy was done, where contact hurts, and whether you have dryness, discharge, bleeding, or urinary symptoms.
