Pain during oral sex: what to know

Burning or rawness on the outside may follow friction, shaving, scented products, thrush or a skin condition such as lichen sclerosus. Pain from light touch can occur with vulvodynia, while deeper pelvic pain may point toward endometriosis, an ovarian endometrioma or pelvic inflammatory disease. Genitourinary syndrome of menopause may cause dryness and fragile tissue after menopause.

Notice whether pain is at the vulva, vaginal opening or deeper in the pelvis. It is also useful to note itching, sores, discharge, odour, bleeding, urinary symptoms and whether a new product or sexual contact came before the discomfort.

Stop if it hurts and rinse gently with lukewarm water. Avoid fragranced soaps, douching and numbing products that can hide worsening irritation. A barrier method can reduce exposure to semen or infections, and regular lubricant may help if dryness contributes. Do not apply steroid or antifungal treatment unless a clinician has advised it; the right treatment depends on the cause.

Book an appointment for repeated pain, visible skin change, itching, sores or pain that lasts after sexual activity. Seek prompt care for fever, lower abdominal pain, foul-smelling discharge, bleeding, severe swelling or difficulty passing urine. A clinician may examine the skin and pelvis and offer infection testing when indicated.

Ask whether the pain is coming from the vulval skin, vaginal tissue or deeper pelvis. Discuss possible infection, skin disease, vulvodynia, endometriosis and menopause-related changes, and ask which products or treatments are safe for you.
