Pain During Intercourse After Hysterectomy: What to Know

After a hysterectomy, tissues need time to heal before vaginal penetration is safe; the timing depends on the operation and your surgeon's advice. Once healing is complete, pain may still occur from dryness, scar sensitivity, pelvic-floor muscle guarding, or deeper pressure. If the ovaries were removed or their hormone production changed, vaginal tissues may become drier and more fragile. Pain can also reflect a condition that led to surgery or another pelvic issue, including endometriosis, adenomyosis, fibroids, or pelvic pain. The symptom does not prove that surgery has failed.

Follow your surgical team's advice about when to resume penetration, and stop if pain or bleeding occurs. Begin gently, allow enough arousal, and consider a suitable lubricant. A clinician may discuss moisturising products, local treatment for vaginal symptoms, pelvic-floor physiotherapy, or review of scar tissue and healing. Record whether pain is at the entrance or deeper, and whether it comes with discharge, bleeding, urinary symptoms, or bowel symptoms. Do not self-treat persistent bleeding.

Seek urgent help for heavy bleeding, severe or sudden pelvic pain, fever, faintness, vomiting, a swollen abdomen, or feeling acutely unwell. Contact your surgical team promptly for new bleeding after sex, foul-smelling discharge, wound concerns, or pain that is worsening. Arrange a review for ongoing pain, because an examination can check healing, vaginal tissues, pelvic muscles, and other causes.
