Pain During First Sex: What to Know

Pain at the vaginal opening may come from not enough arousal or lubrication, involuntary pelvic-floor tightening, skin irritation, or a condition such as vulvodynia or lichen sclerosus. Deeper pain can have different causes, including endometriosis, an ovarian endometrioma, or pelvic inflammatory disease. Anxiety, fear, previous pain, and feeling rushed can increase muscle tension, but pain is not a sign that you have done anything wrong. There is no required amount of bleeding, and the hymen does not reliably show whether someone has had sex.

Consent can be withdrawn at any moment. Stop if it hurts, slow down, and use communication, relaxation, and a water- or silicone-based lubricant. Avoid fragranced products that may irritate vulval skin. Trying non-penetrative intimacy can reduce pressure. If pain returns, arrange an assessment; a clinician can distinguish surface tenderness from deeper pelvic pain and discuss pelvic-floor therapy, treatment for irritation, or other options. Pain during sex is a symptom worth treating, not a test to pass.

Seek prompt care for severe pain, heavy bleeding, fever, unusual discharge, sores, faintness, or pain after a possible injury. Book a routine appointment if pain continues, prevents penetration, causes burning or itching, or leaves you worried. A pelvic examination is your choice, and you can ask the clinician to explain each step and stop at any time.
