Pain Deep Inside During Sex

Deep pain may be linked with endometriosis, an ovarian endometrioma, ovarian or uterine conditions, pelvic-floor muscle tension, or pelvic inflammatory disease. Pain from dryness or delicate tissue changes can occur with genitourinary syndrome of menopause. Lichen sclerosus and vulvodynia more often affect the outer vulval area, but genital discomfort can make penetration painful. The depth, position, timing, and symptoms outside sex help distinguish possibilities.

Pain is not a normal requirement of sex. Fear of pain can cause muscles to tighten further, so stopping when pain begins and seeking a confidential assessment can help break that cycle.

Stop or change activity when pain starts; do not push through it. Note whether pain is deep or at the entrance, whether it occurs with particular positions, and whether bleeding, discharge, urinary symptoms, bowel symptoms, or cycle-related pain accompany it. A longer period of arousal, lubricant, and gentler positions may reduce friction, but recurrent deep pain needs a clinician’s evaluation. Avoid self-treating suspected infection with leftover medicines.

Seek urgent care for severe or sudden pelvic pain, fainting, fever, vomiting, heavy bleeding, or pain with a possible pregnancy. Arrange prompt assessment for new bleeding after sex, foul-smelling discharge, genital sores, or worsening pain. A pelvic infection can require timely treatment, especially when fever or lower-abdominal tenderness is present.

Ask what examination or tests could identify pelvic, vaginal, or muscle causes; whether infection or pregnancy should be checked; and which treatments fit your symptoms. If endometriosis is suspected, ask when referral or laparoscopic excision should be discussed.
