Pain Between Ovulation and Your Period

Some people feel one-sided discomfort around ovulation that settles without treatment. Pain that continues afterward may relate to the ovary, bowel, bladder, pelvic muscles, or uterine lining. Endometriosis can present with pelvic pain that follows a cycle and may also cause painful periods, pain during sex, or bowel and bladder symptoms. An ovarian cyst can cause pressure or aching, and sudden severe pain may occur if it ruptures or twists the ovary.

A late period with pelvic pain can occur in pregnancy, including an ectopic pregnancy, so pregnancy possibility should be mentioned even when bleeding is light or absent.

Track the side, severity, duration, bleeding, bowel or urinary symptoms, and relation to movement or sex. A pregnancy test may be appropriate if pregnancy is possible. Gentle heat and rest may help mild familiar discomfort, but avoid delaying assessment when pain is new, escalating, or interfering with normal activity. Arrange a review for repeated pain between cycles.

Seek urgent care for sudden severe one-sided pain, fainting, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, a rigid or very tender abdomen, or pain with a positive pregnancy test. Prompt assessment is also needed for pain with unusual discharge, pain during urination, or worsening symptoms.

Ask whether the pattern fits ovulation pain, endometriosis, an ovarian cyst, or another cause; whether pregnancy testing or ultrasound is needed; and how to manage pain safely if it recurs. Bring your cycle dates and symptom record.
