Pain Between Knee and Shin: What It Can Mean

Short answer
Pain between the knee and shin may involve the patellar tendon, bursae, kneecap, joint, bone, or nearby soft tissues, and the cause depends on timing and accompanying signs.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Record
- Swelling timing, warmth, locking, giving way, and the activity that triggers pain.
- Protect the knee
- Avoid impact and deep bending while they reproduce symptoms.
Possible sources of the pain
Pain just below the kneecap that follows jumping, running, or repeated bending can involve Patellar Tendinitis. Pain at the front of the knee with stairs, squats, or prolonged sitting may occur with Chondromalacia Patellae. Swelling behind the knee can occur with a Baker's Cyst, while tenderness over a pressure point may involve Knee Bursitis. Joint stiffness can occur with Knee Osteoarthritis; sudden twisting with locking or instability raises concern for a Meniscus Tear or ACL Tear. A hot, very tender joint can occur with Gout.
Note whether the pain began after impact, twisting, increased training, or without an obvious trigger. Record swelling, warmth, clicking, catching, giving way, fever, and whether you can fully straighten or bear weight.
What you can do now
Reduce running, jumping, deep squats, kneeling, and other movements that clearly increase the pain. Gentle range-of-motion work may be comfortable, but do not force a locked or swollen knee. Use a cold pack wrapped in cloth after activity and elevate the leg if it is swollen. Choose supportive footwear and return to loading gradually. If symptoms persist, an examination can identify whether the tendon, kneecap, joint, or another structure needs targeted treatment.
When to seek care
Seek prompt assessment after a significant injury, a popping sensation followed by swelling or instability, inability to bear weight, or inability to straighten the knee. Get urgent help for a hot swollen knee with fever, severe sudden pain, a cold or pale foot, or new numbness or weakness. Arrange routine care when symptoms persist, recur, disturb sleep, or limit stairs, work, or exercise.
Questions for your doctor
Which structure is most tender or restricted on examination? Do I need imaging? Which activities can I continue safely, and when should strengthening begin? What signs would suggest a joint problem rather than a tendon or kneecap problem?

Questions people ask
Does pain below the knee always mean a tendon problem?
No. Bursae, the kneecap, joint, bone, and nearby tissues can produce pain in the same area.
When is knee swelling urgent?
Urgent assessment is appropriate when swelling follows a major injury, the knee is hot with fever, or you cannot bear weight or straighten it.
