Pain after sex during ovulation

Ovulation may make one side of the lower abdomen feel tender, and deeper penetration can bring that discomfort into focus. Pain at the vaginal opening points more towards irritation, dryness, skin changes, or pelvic-floor muscle tightening; deeper pain may involve the cervix, uterus, ovaries, or surrounding tissues. Endometriosis and an ovarian endometrioma can cause deep pain that follows a cycle. Pelvic inflammatory disease may cause pelvic pain with discharge, fever, or pain when the cervix moves. Vulvodynia or lichen sclerosus may cause surface burning or soreness.

The symptom is not a diagnosis. A clinician will consider pregnancy possibility, contraception, infection risk, bleeding, urinary symptoms, bowel symptoms, and whether pain occurs with other types of penetration.

Pause penetration when pain starts and use clear communication about depth and position. A water- or silicone-based lubricant may reduce friction; avoid fragranced washes and products that sting. Keep a cycle diary recording pain location, duration, position, discharge, bleeding, and fever. Arrange an appointment if pain returns, persists after sex, or affects intimacy. Evaluation may include an examination and targeted tests rather than assuming ovulation is the cause.

Seek urgent care for severe or rapidly worsening pelvic pain, fainting, fever, repeated vomiting, heavy bleeding, or pain with a positive pregnancy test. Prompt assessment is also wise when new pain comes with foul-smelling discharge or feeling very unwell.

Is the pain superficial or deep, and what does that suggest? Should I have infection testing, a pregnancy test, or imaging? Could endometriosis or pelvic-floor tension be involved, and what can make sex more comfortable while I am being assessed?
