Ovulation Pain in the Abdomen: What to Know

Ovulation pain, also called mittelschmerz, can arise near the middle of the cycle and may affect the lower abdomen on one side. Some people notice a short twinge; others feel an ache that settles without treatment. Pain that is widespread, continuous, associated with fever or vomiting, or unrelated to cycle timing may have another cause. Endometriosis can cause pelvic pain, and ovarian cysts can sometimes cause pain or pressure; the symptom alone cannot identify either condition.

Record the date, side, character, duration, and intensity of each episode, along with bleeding, discharge, temperature, nausea, bowel changes, and urinary symptoms. Gentle heat, rest, and ordinary activity as tolerated may help mild pain. If episodes are frequent or disruptive, a clinician may review your cycle and perform an abdominal or pelvic assessment. Mention pregnancy possibility before taking medicines or undergoing tests.

Get urgent help for sudden intense abdominal pain, faintness, a rigid or very tender abdomen, heavy bleeding, fever, or repeated vomiting. If pregnancy is possible, pain with bleeding, dizziness, or shoulder-tip pain needs urgent assessment. Make an appointment when pain keeps returning, lasts longer than expected, appears after sex, or changes your normal cycle pattern.
