Ovulation pain and the best time to conceive: what to know

Ovulation pain, or mittelschmerz, can appear around the time an egg is released and may be felt on one side of the lower abdomen. Some people do not feel ovulation, and pain can occur before or after egg release, so it is not a precise fertility test. Fertility depends on the timing of sperm and egg availability, as well as factors affecting either partner. Pain alone cannot confirm ovulation, pregnancy, or a fertility problem. Endometriosis and ovarian cysts may also cause pelvic pain that is unrelated to fertile timing.

If you are trying to conceive, consider combining cycle dates with changes in cervical mucus and an ovulation predictor test. Record pain without treating it as a guarantee of ovulation. Sex during the days leading up to and around suspected ovulation may cover the fertile window. If pain makes sex difficult, discuss symptom relief and evaluation with a clinician. A regular record can also help a doctor assess whether your cycles or pain need further investigation.

Seek urgent care for sudden severe pelvic pain, fainting, fever, repeated vomiting, heavy bleeding, or pain when pregnancy is possible. Arrange a non-urgent fertility or pain review if cycles are difficult to track, intercourse is repeatedly painful, or pelvic pain is persistent or worsening.
