Ovulation pain and fibroids: what to know

Ovulation pain, also called mittelschmerz, may be felt on one side as an ache, cramp, or brief sharper sensation around the middle of a cycle. Fibroids are non-cancerous growths in the muscle of the uterus. They may cause pressure, fullness, painful periods, bleeding between periods, or discomfort during sex, depending on their size and position. Having pain around ovulation does not by itself show that fibroids are present. Keeping a cycle and symptom diary can reveal whether discomfort follows ovulation or continues beyond it. Other pelvic conditions, including endometriosis or ovarian cysts, can produce overlapping symptoms.

Note the day of your cycle, the side and character of the pain, bleeding changes, bowel or bladder symptoms, and how long each episode lasts. Gentle heat and rest may ease mild cramping. If you use an over-the-counter pain medicine, follow its label and check with a pharmacist or clinician if you are pregnant, have stomach or kidney problems, or take other medicines. Arrange a pelvic assessment if pain is recurring, affecting daily activities, or accompanied by heavy periods. An examination and, when appropriate, pelvic ultrasound can assess the uterus and ovaries.

Seek urgent medical help for sudden severe pelvic pain, fainting, marked weakness, fever, repeated vomiting, heavy vaginal bleeding, or pain with a possible pregnancy. These symptoms need prompt assessment because several urgent pelvic conditions can feel similar. Contact a clinician soon if pain is steadily worsening, lasts well beyond the mid-cycle window, or returns with new bleeding or pressure.
