Ovulation pain during sex

Mid-cycle pelvic sensitivity can make deep penetration uncomfortable, especially when an ovary is tender or a cyst is present. Pain at the vaginal opening points toward a different group of causes, such as dryness, vulvodynia, or lichen sclerosus. Deep pain that persists outside ovulation can occur with endometriosis, an ovarian endometrioma, pelvic inflammatory disease, or genitourinary syndrome of menopause. The depth, location, timing, and associated symptoms help guide evaluation.

Stop or change activity when pain begins; continuing through pain can increase guarding and distress. Try slower movement, positions that limit depth, communication with your partner, and a suitable lubricant. Do not insert products that irritate the vulva. Arrange a consultation if pain repeats, causes avoidance, or occurs with bleeding, discharge, dryness, burning, or urinary symptoms. A clinician can discuss treatment options, including evaluation for endometriosis and whether laparoscopic excision of endometriosis is relevant.

Seek urgent care for sudden severe pelvic pain, fainting, heavy bleeding, fever, vomiting, or pain with a possible pregnancy. Prompt review is needed for new pelvic pain with foul-smelling discharge, pain during urination, sores, or feeling acutely unwell. Repeated pain during sex merits a planned assessment even when it appears to follow the cycle.

Tell your doctor whether pain is at the entrance or deep inside, whether it occurs only near ovulation, and whether lubrication or position changes it.
