Ovulation pain before your period

Ovulation pain may happen around the middle of a cycle and can feel like a one-sided ache or brief sharp cramp. In the days before bleeding, hormonal changes can bring pelvic heaviness, bloating, breast tenderness, mood changes, or cramps. Cycle timing is not exact, especially when periods are irregular, so a calendar estimate cannot diagnose the source of pain.

Pain before a period that is unusually intense, lasts beyond bleeding, or occurs between cycles deserves attention. Endometriosis may cause recurring pelvic pain, and ovarian cysts may also cause one-sided symptoms; a clinician can assess these possibilities.

Keep a cycle diary for several months, noting the first day of bleeding, pain onset, location, severity, duration, bowel or bladder symptoms, and bleeding changes. Gentle heat, movement, rest, and suitable over-the-counter pain relief may help. Ask a pharmacist to check medicine safety if you could be pregnant, have stomach or kidney problems, or take regular treatment.

Seek urgent care for sudden severe pain, fainting, heavy bleeding, fever, repeated vomiting, or pelvic pain with a positive pregnancy test. Arrange a review when pain repeatedly stops normal activities, is worsening, appears after sex, or is accompanied by unusual discharge or bleeding.
