Ovulation pain after HSG: what to know

An HSG passes contrast through the cervix and uterus to check the fallopian tubes. Mild cramping soon afterward may relate to the procedure, while pain later in the cycle may reflect an ovary releasing an egg. Ovulation pain is often felt on one side and may last from a short time to part of a day. Timing alone cannot confirm its cause. Pain that continues between cycles, gets stronger, or comes with fever, unusual discharge, vomiting, or heavy bleeding can point to a problem unrelated to ordinary ovulation, including an ovarian cyst or infection. Endometriosis can also cause recurring pelvic pain.

Record the day of pain, its location, how long it lasts, bleeding, discharge, temperature, and any medicine taken. Rest, gentle heat, and fluids may help mild discomfort. Use only pain medicine that your HSG clinician has said is suitable, especially if pregnancy is possible or you have stomach, kidney, bleeding, or allergy concerns. Avoid placing anything in the vagina for the period advised by your clinic after the procedure. Arrange a review if pain does not settle, returns repeatedly, or differs from your usual cycle.

Contact your clinic promptly for worsening pelvic pain, fever, chills, foul-smelling discharge, dizziness, or bleeding that is more than expected after the HSG. Seek urgent care for sudden intense pain, fainting, trouble breathing, or repeated vomiting. If you could be pregnant and have one-sided pain or bleeding, seek urgent medical advice because pregnancy-related causes need to be excluded.

Ask whether the timing fits ovulation, whether an examination or ultrasound is appropriate, and which pain relief is safe for you. Tell the clinician when the HSG happened, whether the tubes were reported as open, and whether you are trying to conceive.
