Ovary pain during sex

Deep pain with penetration can be linked to endometriosis, an ovarian endometrioma, pelvic inflammatory disease, or an ovarian cyst. Pain at the entrance may instead relate to vulvodynia, lichen sclerosus, dryness, or pelvic floor muscle tightening. Genitourinary syndrome of menopause can make vaginal tissues drier and more sensitive. The depth and location of pain, whether it occurs in particular positions, and whether it continues afterward are useful clues. Pain during sex does not identify one diagnosis by itself.

Pause or change activity when pain begins; pushing through it can increase muscle guarding. Note whether the discomfort is superficial or deep, its relation to your period, the position involved, and any bleeding, discharge, urinary symptoms, or bowel pain. A lubricant may reduce friction when dryness contributes, but it will not explain persistent deep pain. Book a gynaecology appointment if pain recurs, affects intimacy, or is accompanied by abnormal bleeding or discharge. Assessment may include a gentle examination, infection testing, pregnancy testing, or pelvic imaging.

Seek urgent care for severe or rapidly worsening pelvic pain, fever, fainting, repeated vomiting, heavy bleeding, or pain when pregnancy is possible. Urgent assessment is also appropriate with new pelvic pain and feeling very unwell. These signs can occur with infection, a pregnancy complication, cyst-related problems, or another condition requiring prompt treatment.

Does the pain sound superficial, deep, or related to the pelvic floor? Could infection, endometriosis, an ovarian endometrioma, or tissue dryness be contributing? Which examination or tests are appropriate, and what can I do while waiting?
