Ovary pain during ovulation on both sides

Ovulation pain, also called mittelschmerz, may feel like aching, pressure, or a brief sharper sensation in the lower abdomen. It can be felt on one side or across the pelvis, and the side may vary from cycle to cycle. Pain on both sides is not proof that both ovaries released an egg. Gas, bowel irritation, bladder problems, pelvic muscle tension, an ovarian cyst, or endometriosis can also cause similar discomfort. The timing, duration, severity, bleeding, discharge, and other symptoms help a clinician sort these possibilities.

Record when the pain starts in relation to your period, where it is located, how long it lasts, and whether it returns in later cycles. Gentle heat, rest, and movement that does not worsen the pain may help. Use an over-the-counter pain medicine only if it is suitable for you and follow its label. Arrange a routine appointment if pain keeps returning, interferes with daily activities, occurs outside the mid-cycle window, or comes with painful periods or pain with bowel movements. Your clinician may consider an examination, pregnancy testing, or pelvic imaging.

Seek urgent medical care for sudden severe pelvic pain, fainting, marked weakness, fever, repeated vomiting, heavy vaginal bleeding, shoulder-tip pain, or pain when pregnancy is possible. These features can signal a problem that needs prompt examination, including pregnancy-related complications, infection, a ruptured cyst, or ovarian twisting. Do not wait for the pain to fit your usual cycle pattern if it is intense or rapidly worsening.
