Can a Pap smear detect ovarian cancer?

A Pap smear, also called cervical cytology, collects cells from the cervix to look for cervical cell changes. The ovaries are separate structures, so a normal cervical result does not assess them directly. Ovarian cancer can present with bloating, pelvic or abdominal discomfort, early fullness, or changes in bowel or bladder habits, although these symptoms also occur with non-cancerous conditions. A Pap result that is abnormal usually concerns the cervix and needs follow-up based on that finding; it does not by itself diagnose ovarian cancer.

Keep attending cervical screening according to local guidance, because it serves a different purpose. If symptoms persist or a clinician feels a pelvic abnormality, assessment may include a history, examination, blood tests, and pelvic ultrasound. Further imaging or tissue sampling may be recommended depending on the findings. If ovarian cancer is diagnosed, treatment planning may include surgery, chemotherapy, genomic tumour profiling, or BRCA genetic testing.

Book a clinical review for persistent bloating, pelvic pain, unusual bleeding, early satiety, or new bowel or urinary changes, even if your Pap smear was normal. Seek urgent help for severe pain, repeated vomiting, fainting, sudden breathlessness, or rapidly increasing abdominal swelling. A screening result should not delay assessment of new or worsening symptoms.
