Osteomyelitis of the lower leg is a bone infection that needs prompt medical assessment, testing and targeted treatment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The short answer

Osteomyelitis of the lower leg is a bone infection that needs prompt medical assessment, testing and targeted treatment. It may follow an open injury, an operation, a nearby skin wound or infection reaching bone through the bloodstream. Lower-leg pain, warmth, swelling, tenderness, drainage or difficulty bearing weight deserve attention, especially when a wound is present.

What osteomyelitis in the lower leg means

The shinbone and nearby tissues can become inflamed when microorganisms enter bone. Some infections develop quickly with marked pain and swelling; others cause persistent discomfort, a slow-healing wound or intermittent drainage. A clinician may examine the skin and circulation, order blood tests and use imaging to assess the bone. A sample from the affected area may help identify the organism and guide medicine.

What to do next

Arrange a medical review rather than treating the problem only with creams or leftover antibiotics. Keep any wound clean and covered, avoid squeezing drainage, and reduce pressure on the leg if walking increases pain. Treatment can include prescribed antimicrobial medicine, wound care, management of blood-flow or blood-sugar problems, and sometimes removal of infected or dead tissue. Bone reconstruction may be considered when infection has left a defect; see Bone Grafting for context.

When to seek urgent care

Seek urgent care for rapidly worsening pain or swelling, spreading redness, pus, a darkening wound, new numbness, a cold or pale foot, confusion, faintness, or difficulty moving the ankle or toes. Fever or chills can occur, but their absence does not exclude a bone infection. A recent fracture, surgery, deep wound or implanted hardware lowers the threshold for prompt assessment.

Questions for your doctor

Ask which bone and tissue appear involved, whether imaging or a sample is needed, how long treatment may take, whether weight-bearing should be limited, and how the wound will be monitored. Clarify which symptoms should trigger same-day review and how to protect the leg during recovery.

Questions people ask

Can lower-leg osteomyelitis occur without a wound?

Yes. Infection can reach bone through the bloodstream, so an obvious skin opening is not required.

Can I walk with a lower-leg bone infection?

Walking may worsen pain or stress damaged tissue; follow the clinician’s instructions about support and weight-bearing.
