What should you know about osteomyelitis in sickle cell disease?

Sickle cell disease can reduce blood flow to bone and can also affect immune defence, so infection may develop in bone. Osteomyelitis may cause persistent or localised bone pain, tenderness, swelling, warmth, fever, or reduced movement. Symptoms can overlap with a vaso-occlusive pain episode, and both problems can occur together.

Assessment may include an examination, blood tests, cultures, and imaging. The clinician considers the painful area, fever pattern, previous infections, and usual sickle symptoms. A normal-looking skin surface does not exclude deeper infection.

Contact your sickle cell team or urgent medical service if pain is new, focused in one area, persistent, or accompanied by fever. Do not rely on leftover antibiotics. If treatment is prescribed, take it for the instructed duration and report rash, severe diarrhoea, or worsening symptoms.

Continue your individual sickle cell plan for fluids, pain relief, and other medicines unless a clinician changes it. Keep notes on temperature, pain location, movement, and fatigue so the team can compare the illness with your usual pattern.

Seek urgent help for fever, shaking chills, severe or rapidly increasing pain, new swelling, inability to use a limb, confusion, breathing difficulty, fainting, or marked weakness. Children with sickle cell disease and fever need prompt assessment because infection can progress quickly.

Ask which tests can distinguish infection from a pain crisis, whether imaging or a culture is needed, how treatment will be monitored, and which symptoms should trigger immediate return.
