What should you know about oligoprogression lung cancer?

Your oncology team may identify this pattern on follow-up imaging. It is different from growth in many sites at once, because some areas may still be responding or stable. The growing sites can be in the lung or elsewhere, including the brain; symptoms depend on their location. A hoarse voice, for example, can have many causes and does not by itself establish cancer progression.

The team may compare current scans with earlier images, review your symptoms and check whether the cancer has changed in a way that affects treatment choices. Genomic tumour profiling may help identify a tumour change that could influence targeted treatment.

Keep scheduled scans and tell your oncology team about new or changing symptoms. Ask whether the scan findings represent oligoprogression, treatment-related change or another explanation. Depending on the number and position of growing sites, options may include continuing an effective systemic treatment while treating selected areas with radiotherapy, surgery or another local approach. Chemotherapy may be considered when the wider pattern or tumour features make it appropriate.

The plan should account for previous treatments, overall health, lung function, the location of each site and your priorities. Palliative care can support breathlessness, pain, fatigue and emotional strain at any stage alongside cancer treatment.

Contact your cancer team promptly for worsening breathlessness, chest pain, coughing blood, a new hoarse voice, persistent severe headache, new weakness, confusion, a seizure or difficulty walking. Seek emergency help for severe breathing difficulty, sudden neurological change or chest pain that does not settle. These symptoms need assessment because several urgent conditions can cause them; they do not prove that cancer has progressed.

Which sites have changed, and how certain is that interpretation? Could a biopsy or genomic tumour profiling change the plan? Can focused radiotherapy or surgery treat the growing areas while the current medicine continues? What symptoms should trigger an urgent call?
