Olecranon stress fracture: what to know

How the structures involved in Stress Fracture differ from normal

Short answer

An olecranon stress fracture is a crack caused by repeated loading at the elbow tip and needs assessment because continued use can worsen bone injury.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Pain pattern
Focal tenderness at the elbow tip that may worsen with pushing or straightening
Until reviewed
Stop painful loading and avoid repeatedly testing the elbow

How an olecranon stress fracture develops

The olecranon is the pointed part of the ulna at the back of the elbow. Repeated force from throwing, pushing, falls onto the elbow, or forceful muscle pull can overload the bone before it has time to recover. Pain is often focused at the elbow tip and may increase with pushing, straightening, or activity. Swelling or tenderness can occur, but symptoms vary. Bone-health problems, including Vitamin D Deficiency, may be relevant and should be reviewed rather than assumed to be the cause.

A clinician may ask about training or work demands, examine the exact tender area, and use imaging. An early stress injury may not be obvious on an initial image, so follow-up is sometimes needed. Other stress fractures can cause [Foot Pain](/symptoms/foot-pain/), [Heel Pain](/symptoms/heel-pain/), [Shin Pain](/symptoms/shin-pain/), or [Thigh Pain](/symptoms/thigh-pain/), but symptoms in those areas do not diagnose an elbow injury.

What to do before assessment

Stop throwing, lifting, pushing, and exercises that reproduce pain. Keep the elbow protected and avoid testing it repeatedly. Do not return to sport because the pain briefly settles. A clinician may recommend a brace, altered activity, or a staged rehabilitation plan; follow the individual instructions. Ask before starting supplements, including [Vitamin D Supplementation](/treatments/medical-management/vitamin-d-supplementation/), because the right choice depends on testing, diet, medicines, and health history.

When care is urgent

Arrange prompt assessment for focal elbow-tip pain that persists or returns with activity, especially after a sudden increase in loading. Seek urgent care after a fall or direct blow if the elbow looks deformed, movement is severely limited, or pain is intense. Numbness, a pale or cold hand, inability to move the fingers, or rapidly increasing swelling needs immediate attention. [Stress Fracture](/conditions/stress-fracture/) explains the broader injury pattern; do not use a general page to replace an elbow examination.

Questions for your clinician

Ask which activity caused the load, whether imaging or repeat imaging is needed, and how the elbow should be protected. Clarify safe movement, work and sport restrictions, bone-health tests, pain relief options, and the signs that mean the injury is not settling.

Questions people ask

Can an olecranon stress fracture happen without a major accident?

Yes. Repeated loading can gradually exceed the bone’s ability to recover, even without one dramatic injury.

Can I keep training if the pain is mild?

No. Continuing painful loading may worsen a stress injury. Stop the provoking activity and arrange clinical assessment.
