Ocular migraine without headache: what it means

A person may see flashing lights, shimmering edges, zigzag shapes, or a patch of missing vision and then feel well once it passes. The absence of head pain does not rule out migraine aura. However, visual disturbances can also arise from an eye problem, circulation problem, infection, dehydration, heat illness, or another neurological condition. The exact timing, affected area, and accompanying symptoms help a clinician decide what needs checking.

Sit or lie somewhere safe, avoid driving, and write down the start and end of the episode. Covering one eye at a time may help you describe whether the disturbance is present in one eye or in the same side of both visual fields, but do not delay urgent care to perform this check. Arrange a medical review for a first episode, a changed pattern, or repeated symptoms. Maintain fluids and regular meals, especially after heat exposure or illness.

Call emergency services for sudden vision loss, facial drooping, arm weakness, speech trouble, confusion, collapse, or a severe unfamiliar headache. Seek urgent eye care for pain, redness, or a curtain-like shadow in the vision. Prompt assessment is also needed when symptoms last longer than usual, affect only one eye, or do not fully clear.

Ask whether the pattern is consistent with migraine aura, whether you need an eye examination or other tests, and which changes should trigger emergency action. Mention recent fever, respiratory illness, poor fluid intake, heat exposure, stress, and any new medicine.
