How they can overlap

Postpartum OCD may involve unwanted, frightening thoughts about accidental harm, contamination, or checking the baby. Compulsions can include repeated checking, washing, seeking reassurance, or avoiding situations. These thoughts are distressing and do not by themselves mean a parent wants to act on them. Postpartum depression can bring persistent sadness, emptiness, guilt, loss of interest, sleep or appetite changes, and difficulty feeling connected.

The conditions may reinforce each other: low mood can make intrusive thoughts feel more threatening, while compulsions can consume time and increase exhaustion. A clinician can ask about the thoughts, urges, behaviours, mood, sleep, and safety without judgement.

Accept practical help with feeds, meals, rest, and appointments. Tell a trusted person what you are experiencing so you are not carrying it alone.

When to seek urgent help

Seek urgent help if you may harm yourself or the baby, feel unable to stay safe, hear or see things others do not, feel unusually confused, or believe you must act on a frightening thought. Ask someone to stay with you and contact emergency services or the nearest emergency department.
