What should you know about nodules on back of neck?

A lump at the hairline can arise in the skin, a hair follicle, or tissue beneath the skin. A boil is often tender, warm, and red, sometimes with a central pocket of pus. A blocked cyst may feel smooth and rounded. A lymph node can enlarge during or after an infection. Less commonly, a persistent skin growth such as basal cell carcinoma needs assessment. The lump itself does not establish its cause.

Location, movement, tenderness, surface changes, and duration help the clinician narrow the possibilities. Mention recent infections, shaving or friction, bites, scalp problems, and any medicines that affect immunity.

Do not squeeze, pierce, or repeatedly massage the nodule. Keep the area clean, reduce friction from collars or straps, and use a warm, clean compress for comfort if the skin is sore. Arrange a review if it persists, recurs, enlarges, or cannot be clearly linked to a short-lived irritation.

A clinician may inspect the skin and nearby lymph nodes, and may recommend treatment for infection, drainage for a suitable boil, or another procedure based on the diagnosis. Intralesional steroid injection is sometimes used for selected inflammatory skin nodules, but it is not appropriate for every lump.

Seek same-day care for rapidly increasing swelling, severe pain, spreading redness, fever, or difficulty swallowing or breathing. Arrange prompt assessment for a lump that bleeds, crusts, ulcerates, becomes fixed, or remains without explanation.

Ask whether the lump is in the skin or a lymph node, whether it needs imaging or sampling, and which changes should prompt review. Ask whether home care is suitable and whether an injection or other treatment could obscure the diagnosis.
