Strep throat without tonsils: what to know

Tonsil removal may reduce problems involving the tonsils, but it does not remove all tissue that can be infected by streptococcal bacteria. A sore throat, fever, painful swallowing, and swollen glands in the neck can occur. Cough, runny nose, or hoarseness may point more toward a viral illness, but symptoms overlap, so appearance alone cannot confirm the cause.

A clinician may examine your throat and decide whether a throat swab or other test is appropriate. A sore throat after surgery is not automatically strep throat, and a diagnosis should not be made from the absence of tonsils alone.

Rest, drink fluids, and choose foods that are easier to swallow. Use only pain relief that is suitable for you and follow the package or prescribing directions. Arrange a GP consultation if pain is significant, fever is present, or symptoms are not settling. If strep throat is diagnosed, take antibiotic treatment exactly as prescribed and do not share leftover medicine.

Get urgent medical help for breathing difficulty, drooling because you cannot swallow, rapidly increasing throat or neck swelling, confusion, or severe dehydration. Seek prompt assessment for a marked one-sided throat pain, a muffled voice, or difficulty opening the mouth. These signs can indicate a complication requiring examination.

Ask whether your symptoms warrant a strep test, how to manage pain while waiting, when you can return to work or school, and what changes should trigger urgent review. Mention allergies, recent antibiotics, and the date of tonsil surgery if relevant.
