How the condition behaves

Neurally mediated hypotension is a reflex response in which blood pressure falls, often around prolonged standing, heat, emotional stress, pain, or a medical procedure. The fall may reduce blood flow to the brain and produce dizziness, nausea, sweating, blurred vision, weakness, or fainting. A diagnosis requires looking at the event pattern and excluding other explanations, including dehydration, heart-rhythm problems, low blood sugar, or anaemia. Treatment therefore differs between people.

Treatment and prevention

At the first warning, sit or lie down safely and raise your legs if possible. Rise gradually, avoid standing still for long periods, keep cool, drink fluids, and follow the salt advice given by your clinician rather than changing intake independently. Some people benefit from leg-muscle tensing or compression garments. A clinician may review blood-pressure medicines and, when lifestyle measures are insufficient, consider prescribed treatment. Do not stop medicines without guidance.
