Understanding the pattern

The neck and shoulder girdle work together, so prolonged computer use, stress, carrying a load or an awkward sleeping position can tighten both areas. Notice whether the restriction is mainly turning the head, lifting the arm, reaching behind the back, or all of these. Painful loss of shoulder movement may occur with frozen shoulder, while pain during lifting can occur with a rotator cuff tear. Shoulder osteoarthritis may cause aching, stiffness and reduced range. Keep both the neck and shoulder moving gently within comfortable limits. Take frequent posture breaks, support the forearms when working, and avoid repeatedly lifting through sharp pain. Heat may relax tight muscles; use a cloth barrier and stop if skin becomes irritated. Do not force the shoulder through a blocked range or repeatedly test painful strength. A physiotherapist can compare neck and shoulder movement, check strength, and build a gradual mobility plan.

When to seek care

Arrange assessment when stiffness lasts, worsens, follows a fall, or interferes with dressing, sleep or work. Seek urgent care for a visibly deformed shoulder, sudden inability to lift the arm after injury, new arm weakness or numbness, chest pressure, breathlessness, sweating, or pain spreading to the jaw or arm. Fever with a hot, swollen joint also needs prompt evaluation.
