What is happening
The navicular sits between the ankle and forefoot and carries force through the arch. Repeated impact, a rapid training increase, hard surfaces, or insufficient recovery can produce a stress injury. Pain may be localised to the midfoot and become sharper with running or jumping. Some people develop a limp, while others have symptoms only during activity.
A normal early X-ray does not always settle the question. Your clinician may use the examination, training history, and further imaging when needed. Bone health, nutrition, and vitamin D deficiency can be relevant contributors, but they are not assumed without assessment.
Management and return
Pause running and jumping, and follow instructions about a boot, cast, crutches, or non-weight-bearing. Do not test the injury by repeatedly hopping or running. Maintain fitness with approved alternatives that do not provoke pain. Later stages may include calf and foot strengthening, balance work, and a measured return to impact. Vitamin D supplementation should follow a clinician’s advice rather than self-prescribing high doses.
When to seek care
Book an assessment for persistent focal foot pain or pain that keeps returning after training changes. Seek urgent help if you cannot bear weight after an injury, or if severe swelling, deformity, numbness, or pale or blue toes occur.
