Nausea and heartburn before labor: what to know

As pregnancy advances, the growing uterus can press on the stomach, while hormonal changes may relax the valve that normally keeps stomach contents down. This can cause burning behind the breastbone, sour fluid in the mouth, early fullness, or queasiness. Heartburn is also called acid reflux; gastritis, a hiatus hernia, or a peptic ulcer can cause similar upper-abdominal symptoms. These symptoms do not by themselves show that labor has started.

Notice whether symptoms follow meals, lying down, or particular foods. Tell your maternity team about persistent vomiting, new upper-abdominal pain, headache, vision changes, or swelling, because those features need assessment in pregnancy.

Try small, regular meals and eat slowly. Remain upright after eating, avoid lying down soon after meals, and raise your upper body for sleep if your maternity clinician says this is suitable. Choose fluids in frequent sips if nausea makes drinking difficult. Avoid foods or drinks that clearly trigger burning, and do not take an antacid or acid-reflux medicine unless a pharmacist, doctor, or maternity clinician has checked that it is appropriate for you. Keep a note of vomiting, food triggers, and fetal movement.

Seek urgent maternity advice for repeated vomiting with difficulty keeping fluids down, blood in vomit, black stools, severe or persistent abdominal pain, chest pain, fainting, breathlessness, or reduced fetal movement. Contact your maternity unit promptly for a severe headache, visual disturbance, sudden swelling, or pain under the ribs. If you think your waters have broken, contractions are regular, or there is vaginal bleeding, follow your local labor assessment instructions.

Ask which food changes are sensible for you, whether your symptoms fit acid reflux, and which pregnancy-safe options could relieve them. Also ask how vomiting or reduced intake might affect hydration, what warning signs should send you to the maternity unit, and whom to call outside clinic hours.
