Nappy rash and yeast infection: what to know

Yeast can overgrow in a warm, damp, covered area and cause a red, sore, itchy rash. Skin folds may be involved, and small spots can appear around the main irritated area. Irritant dermatitis from moisture, friction, soaps, or stool can look similar, so a rash is not proof of infection. In an adult, genital symptoms may include vaginal itching, vulval pain, or thick vaginal discharge; these symptoms also have other causes. A clinician may need to examine the skin or take a sample when the diagnosis is unclear or symptoms recur.

Change nappies or damp clothing promptly, rinse gently with lukewarm water, pat the area dry, and allow short periods of air exposure when practical. Avoid fragranced wipes, harsh cleansers, and scrubbing. Use a clean, absorbent covering and follow the directions for any barrier product. Do not share towels. If a clinician confirms yeast, antifungal treatment for thrush may be recommended in an appropriate form; use it exactly as directed and complete the advised course. Stop and seek advice if the skin becomes more irritated after a product.

Arrange a medical review if the rash is spreading, broken, blistered, very painful, repeatedly returning, or not improving with careful skin care. Seek prompt help if there is fever, marked swelling, pus, unusual discharge, or a child appears unwell or is feeding poorly. Get urgent care for rapidly worsening redness, severe pain, or signs of dehydration. The need for assessment is guided by these warning features rather than by assuming every nappy rash is yeast.
