Nail Ridges and Psoriasis: What You Should Know

Nail ridges can occur when psoriasis affects the nail matrix, the area that forms the nail, and they may appear with pits, discolouration, separation, or debris beneath the nail. A ridge is not specific to psoriasis, so a clinician may also consider injury, fungal infection, eczema, or other nail conditions before deciding on treatment. Changes can affect fingernails, toenails, or both.

Psoriasis may involve the nail matrix, which can produce pits, grooves, roughness, or irregular growth as the nail forms. It may also affect the nail bed, causing the nail to lift from the skin underneath, develop yellow-brown colouring, or collect scale-like material. Some people have nail changes without an obvious skin rash, while others also notice scaly plaques, redness, cracking, peeling, dryness, or itch. The appearance alone cannot confirm the cause. A clinician may inspect the nails and surrounding skin, ask when the change began, and check for tenderness, discharge, trauma, or a pattern affecting several nails. A sample may sometimes be needed when infection is a possibility.

Arrange a dermatology or primary-care assessment if ridges persist, spread, or occur with lifting, pits, colour change, pain, or swelling. Keep nails trimmed but avoid cutting into the skin, scraping beneath the nail, biting, or forcibly removing a lifted edge. Wear gloves for repeated wet work or contact with cleaning products, and moisturise the hands and nails after washing. Do not apply a steroid, antifungal, or other medicated product to the nail unless a clinician has advised it, because the right treatment depends on whether psoriasis, infection, or injury is responsible.

If psoriasis is confirmed, treatment may target the nail itself, nearby skin, or more widespread inflammation. Nail growth is slow, so improvement is judged by healthy nail growing from the base rather than by an instant change to the existing nail.

Ask: Could this be nail psoriasis, an infection, or an injury? Do the changes suggest involvement of the nail matrix or nail bed? Should the nail be tested before treatment? Which care is suitable for the affected nail, and what signs would mean I should return sooner? Mention any joint stiffness or swelling, because psoriasis can affect joints as well as skin and nails.
