Myalgia rheumatica: what should I know?

Aching may follow unusual exercise, poor sleep, dehydration, or a viral illness. COVID-19 and Influenza can present with widespread muscle aches alongside fever, cough, sore throat, or tiredness. Malaria and Dengue Fever are considerations after relevant travel, especially when fever, chills, headache, or marked weakness is present. Fibromyalgia can cause widespread pain with unrefreshing sleep, while an underactive thyroid may bring aches with cold intolerance, constipation, or low energy. The symptom pattern, examination, medicines, and travel history help separate these possibilities.

Rest from strenuous activity but keep gentle movement if it does not worsen the pain. Drink fluids, note fever or rash, and record where the aching occurs and whether stiffness is worse after rest. Review new medicines with a clinician rather than stopping prescribed treatment abruptly. Arrange an appointment when pain persists, limits normal activity, keeps returning, or comes with weakness, swollen joints, or prolonged morning stiffness.

Seek urgent care for severe weakness, trouble breathing, chest pain, confusion, a stiff neck, very high fever, dark urine after intense exertion, or inability to walk. After travel to a malaria area, fever with chills needs prompt assessment. A rapidly spreading rash, facial swelling, or severe muscle pain after a new medicine also warrants urgent advice.
