My voice is very hoarse: what should I know?

Short answer
A very hoarse voice is commonly caused by inflammation or irritation of the vocal cords, but persistent or severe changes need assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common triggers
- Colds, voice strain, smoke, dry air, and reflux irritation
- Voice care
- Use a comfortable speaking voice and avoid whispering
What it may mean
Hoarseness can make your voice rough, breathy, weak, or lower than usual. A Common Cold may inflame the throat and voice box, while shouting, singing, frequent throat-clearing, smoke, or dry air can strain the vocal cords. Acid Reflux may irritate the voice box, sometimes with a sour taste or symptoms after lying down. Less common causes include an underactive thyroid or a problem affecting the throat, thyroid, or lungs. A hoarse voice is a common presentation of these conditions when they affect the relevant structures; it does not by itself identify the cause.
What to do now
Rest your voice by speaking briefly and at a comfortable volume; avoid whispering, shouting, and singing. Drink fluids, avoid smoke and vaping, and use moist air if your room is dry. Do not repeatedly clear your throat. If reflux symptoms accompany the change, mention them to a clinician rather than starting long-term treatment on your own. Arrange a routine appointment if the hoarseness does not settle, keeps returning, or interferes with work or swallowing.
When to seek care
Seek urgent help for difficulty breathing, noisy breathing, inability to swallow saliva, rapidly increasing throat swelling, or a blue or grey colour around the lips. Get prompt medical advice for coughing blood, a neck lump, unexplained weight loss, persistent ear pain, or hoarseness that continues. These warning signs can occur with serious throat or chest conditions and should not be judged by the sound of the voice alone.
Questions for your doctor
When did the voice change begin, and could infection, voice strain, reflux, medicines, or smoking be contributing? Do I need an examination of my throat or vocal cords? What symptoms should make me return sooner?

Questions people ask
Can a cold make my voice hoarse?
Yes. Inflammation from a cold can temporarily affect the vocal cords.
Should I whisper while my voice recovers?
Whispering can strain the voice, so quiet, relaxed speech is usually preferable.
