My scalp is itchy but has no dandruff: what should I know?

A product reaction can cause itch, burning, or redness before scaling appears. Head lice may cause intense itch with tiny nits close to the hair roots rather than obvious dandruff-like flakes. Seborrheic dermatitis can be subtle, and folliculitis may show small bumps instead of scale. Scalp psoriasis can begin as a defined itchy area. Dryness, sweat, heat, tight headwear, or repeated scratching can also irritate the scalp.

Dandruff is not ruled in or out by itch alone. Compare flakes: dandruff and flaky scalp usually shed, whereas nits cling to hairs. A clinician can inspect the scalp with magnification when the findings are difficult to see.

Stop recently introduced dyes, fragrances, oils, and styling products, and use a gentle shampoo while the scalp settles. Rinse well, avoid hot water and scratching, and keep nails short. Do not comb aggressively or use lice treatment simply because the scalp itches; check for nits or obtain advice first. If symptoms continue, note the location of itch, any redness or bumps, and whether others at home are affected.

Arrange a medical review for persistent itch, sleep disruption, recurrent sores, or new hair loss. Seek prompt care for pus, painful swelling, fever, spreading redness, thick crusts, or tender neck glands. Facial swelling or breathing difficulty requires emergency assistance.

Ask whether the scalp needs examination for lice, seborrheic dermatitis, folliculitis, psoriasis, or contact irritation; whether a test is useful; and which shampoo or topical treatment fits the findings. Explain every recent product change and any household itching.
