What the pattern may mean

A spasm can occur after lifting, twisting, prolonged sitting, or a sudden change in activity. Muscle strain commonly causes local pain and guarding. Facet joint syndrome may produce pain with certain back movements, while a herniated disc can irritate a nerve and send symptoms into the buttock or leg. Non-specific low back pain is also common when no single tissue explains the symptoms. What to do during a spasm

Move slowly into a comfortable position, breathe steadily, and avoid forcing the back straight. Brief heat may relax tight muscles for some people; protect the skin and stop if it aggravates symptoms. Gentle walking and small comfortable movements can be preferable to staying still for a long time. Once the episode settles, a physiotherapist can assess movement control and may discuss options such as myofascial release or dry needling when appropriate. Do not use treatment to mask worsening neurological symptoms.

When to seek care

Seek emergency help for loss of bladder or bowel control, numbness around the saddle area, severe or worsening leg weakness, or back pain after major trauma. Obtain prompt assessment for fever, unexplained weight change, pain that is constant at night, or repeated spasms with spreading numbness. Arrange a routine review if episodes keep returning, restrict work or sleep, or do not settle with sensible activity changes.
