The short answer

A groin muscle strain can cause nearby testicular discomfort, but sudden or severe testicular pain needs urgent assessment. Pain may follow lifting, running, twisting, or another movement, yet the testicle itself can also develop problems unrelated to muscle injury.

What it may mean

A strained groin muscle often hurts when you walk, turn, cough, or squeeze the legs together, and may feel tender in the inner thigh or groin. Testicular swelling, warmth, urinary burning, discharge, fever, nausea, or pain that began without exertion suggests another explanation. Inguinal hernia, chlamydia, gonorrhoea, mumps, and twisting of the testicle are among the conditions a clinician may consider. Testicular pain alone cannot identify its cause.

What you can do

Stop strenuous activity and support the scrotum with close-fitting underwear. A wrapped cold pack applied briefly may reduce discomfort after a suspected strain; do not place ice directly on skin. Avoid massage, heavy lifting, and sexual contact until the cause is clearer if infection is possible. Do not rely on self-care when pain is new, one-sided, swollen, or persistent.

When to seek care

Go to an emergency department for sudden intense testicular pain, a high-riding or twisted-looking testicle, rapid swelling, nausea or vomiting, or pain after an injury. Prompt assessment is also needed for fever, urinary symptoms, discharge, or a groin bulge that becomes painful or cannot be pushed back. These patterns can require time-sensitive treatment.

Tell the clinician when the pain started, whether it followed exercise or injury, and whether it changes with movement. Mention swelling, urinary symptoms, discharge, fever, sexual exposure, a groin lump, and any history of undescended testicle or prior surgery.
