Muscle pain in the flank area

Muscle pain in the flank area often follows movement or strain, but urinary symptoms, fever, or severe waves of pain need medical assessment. Flank pain is felt between the ribs and hip on the side of the body and may involve muscles, the spine, kidneys, or nearby tissues.

Pain that changes with twisting, lifting, coughing, or pressing the area is more consistent with a muscle or body-wall source. Kidney stones may cause intense pain that comes in waves and can spread toward the groin. Kidney infection may cause flank or back pain with fever, chills, nausea, or painful urination. A urinary tract infection can cause burning, urgency, or frequent urination, although flank pain makes assessment more important.

Pause lifting and other movements that trigger the pain, but keep gently mobile if that feels comfortable. Use a comfortable position and consider a warm pack for a suspected muscle strain. Drink normally rather than forcing fluids, and record the side, onset, triggers, urine changes, and any fever. A clinician can examine the abdomen, back, and urine to separate muscle pain from urinary causes.

Seek urgent care for severe or rapidly worsening pain, fainting, confusion, repeated vomiting, inability to pass urine, or blood in urine. Get prompt medical advice for flank pain with fever, chills, painful urination, new urinary urgency, or marked tenderness. Pregnancy, one functioning kidney, or a history of kidney disease warrants earlier contact with a clinician.

Ask whether the pain behaves like a muscle strain or a urinary problem, whether urine testing or imaging is needed, and which changes should prompt urgent review.
