What the pain may mean

Pain after running, lifting, climbing, or a sudden change in activity may come from a gluteal muscle strain. Tenderness over the outer hip, especially when lying on that side or climbing stairs, can occur with gluteal tendinopathy. Deep pain with tingling, burning, or pain travelling down the leg can involve nerve irritation such as sciatica or piriformis syndrome. Pain close to the dimples of the lower back may be related to the sacroiliac joint. These patterns overlap, and buttock pain does not identify one condition by itself.

Reduce the movement that clearly increases pain for a short period, while keeping gentle walking if it is comfortable. Avoid long uninterrupted sitting and change position regularly. Cold can soothe a recently irritated area; warmth may feel better for stiffness. Once pain settles, gradual strengthening of the hip and trunk can help restore tolerance. A physiotherapist can check hip movement, spinal and pelvic mechanics, and nerve-related signs before choosing exercises. Do not force stretches that reproduce sharp or travelling pain.

Arrange an assessment if pain persists, repeatedly returns, limits walking or sleep, or follows a significant fall. Seek urgent medical help for new loss of bladder or bowel control, numbness around the inner thighs, marked leg weakness, fever with a hot swollen area, or sudden severe pain with inability to bear weight.
