Why it may happen

Sex can involve sustained hip, thigh, abdominal, pelvic, or calf positions. Holding a muscle shortened for a while, changing position quickly, or exerting yourself after little activity can trigger a spasm. Heat, sweating, dehydration, and a previous muscle strain may add to the problem. Repeated cramps can also be associated with medicines or conditions such as chronic kidney disease, hypothyroidism, vitamin D deficiency, sarcopenia, or peripheral arterial disease. What to do in the moment

Pause and choose a comfortable position. Breathe slowly, then gently lengthen the affected muscle without bouncing or forcing the joint. A lubricant, slower position changes, and avoiding prolonged pressure on one limb may reduce repeat episodes. Never continue through severe pain.

Seek urgent care for severe or persistent pelvic or abdominal pain, fainting, heavy bleeding, fever, chest pain, breathlessness, or a swollen and painful limb. Contact a clinician if cramps recur, occur with weakness or numbness, or make sexual activity difficult.
