Short answer

A muscle cramp behind the knee is a sudden tightening that often improves with gentle stretching, movement, and fluids.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical feeling
A hard, painful tightening behind the knee
First response
Gentle stretch, light massage, warmth, and fluids
Check promptly
Swelling, redness, warmth, weakness, or breathing symptoms

What it can mean

The spasm may involve the calf or hamstring muscles and can follow exercise, prolonged sitting, heat, or fluid loss. A muscle strain can leave soreness after the spasm settles. Dehydration, some medicines, and changes in minerals can also contribute. Less commonly, recurring cramps may occur alongside chronic kidney disease, hypothyroidism, vitamin D deficiency, sarcopenia, or peripheral arterial disease. A cramp itself does not identify one underlying condition.

What to do during a cramp

Stop the activity and straighten the leg gradually. Pull the toes toward your face if the calf is tight; for a hamstring spasm, keep the knee gently extended without forcing it. Light massage, warmth, and slow walking may help after the muscle releases. Drink fluids, especially after sweating, and avoid intense exercise until normal movement returns. Do not press hard or stretch into sharp pain.

When to seek care

Arrange medical advice if cramps keep returning, disturb activity or sleep, last unusually long, or leave persistent weakness. Seek urgent help for a swollen, red, warm leg; sudden breathlessness or chest pain; inability to bear weight; or severe pain after an injury. A clinician can examine circulation, nerves, medicines, and possible muscle injury.

Questions for your doctor

Ask whether your medicines, activity pattern, hydration, or a health condition could be contributing. Mention which muscle tightens, how often it happens, what you were doing, and whether swelling, numbness, or weakness follows.

Questions people ask

Can walking help a cramp behind the knee?

Slow walking may relax the muscle after gentle stretching, but stop if pain intensifies or the leg cannot support you.

Should I use heat or ice?

Warmth often helps an active spasm; cold may soothe soreness after an injury. Protect the skin and avoid either if sensation is reduced.
