Muscle aches, sore throat and fatigue: what to know

A sore throat plus fatigue and muscle aches commonly occurs when the immune system is responding to an infection. Influenza and COVID-19 may also cause fever, cough, headache, or loss of appetite. Dengue fever, malaria, and brucellosis can cause aches and fever after relevant travel or exposure. The presence of these symptoms does not identify one diagnosis by itself.

Fibromyalgia and hypothyroidism can cause fatigue and body aches, but a new sore throat may point to a separate problem. Consider the onset, temperature, hydration, exposure to illness, travel, and any rash or breathing change.

Rest your voice and body, sip fluids, and choose soft foods if swallowing is uncomfortable. Warm drinks, salt-water gargles, or a humidified room may soothe the throat. Stay home from close contact while you feel acutely unwell and follow local advice about testing when infection is a concern.

Use medicines only as directed and check with a pharmacist if you are pregnant, have liver or kidney disease, or take regular treatment. If symptoms are not improving, or fatigue is interfering with normal activities, arrange a clinical review.

Seek urgent care for difficulty breathing or swallowing, drooling, inability to keep fluids down, confusion, severe dehydration, a stiff neck, or rapidly worsening illness. Seek prompt advice for a persistent high temperature, a new widespread rash, unusual bleeding, severe weakness, or recent travel to a malaria-risk area. Contact a clinician if the sore throat or fatigue persists or repeatedly returns.
