Mittelschmerz and pregnancy

Mittelschmerz usually occurs near ovulation, before conception could be established. Once pregnancy is possible, one-sided or persistent pelvic pain should not be casually assigned to ovulation. A missed period, positive pregnancy test, bleeding, dizziness, or shoulder-tip pain changes the urgency. Cysts, infection, miscarriage, or an ectopic pregnancy can produce pelvic pain in pregnancy-related situations. A negative test taken early may not settle the question if the period remains absent.

If pregnancy is possible, use a home test according to its instructions and repeat or seek clinical advice when the result does not fit your symptoms. Keep track of pain location, bleeding, timing, and severity. Use only pain relief considered suitable when pregnancy is possible, and check with a pharmacist or clinician before taking medicines. Arrange assessment for pain that persists, returns, or is accompanied by discharge or urinary symptoms.

Seek emergency care for severe or worsening one-sided pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, or pain with a positive pregnancy test. Do not wait for the pain to become unbearable. These signs can indicate a pregnancy complication or another condition requiring prompt treatment.

Ask when to repeat a pregnancy test, which pain medicines are appropriate, and whether you need an examination or ultrasound. Tell the clinician the first day of your last period, contraception used, test results, and any bleeding.
