Why the pattern can change
Perimenopause is the transition before menopause. Hormone levels and ovulation may vary, causing cycles to become shorter, longer, heavier, lighter, or occasionally skipped. Pelvic pain that follows a changing cycle may still be ovulation-related, but cycle timing is less useful as a guide. Fibroids, ovarian cysts, endometriosis, infection, bowel problems, and urinary conditions can also cause similar symptoms. A symptom diary can show whether pain is linked with bleeding or other changes.

Use gentle heat, rest, and a pain medicine that is suitable for your health history. Record the location, duration, intensity, bleeding pattern, hot flushes, bowel or bladder symptoms, and any medicines or hormone treatments. Book a routine review when pain is recurrent, affects daily life, or appears alongside a new bleeding pattern. A clinician can decide whether examination, pregnancy testing, infection checks, or imaging is appropriate.
Warning signs
Seek urgent care for sudden severe pain, fainting, heavy bleeding, fever, repeated vomiting, or a swollen or rigid abdomen. Bleeding after twelve months without a period also needs prompt medical review. Contact a clinician sooner for pain during sex, unusual discharge, or pain that steadily worsens. These features require assessment rather than attribution to perimenopause.
