What mittelschmerz means

During ovulation, an ovary releases an egg. Stretching of the ovarian surface, fluid released from the follicle, or irritation of nearby tissue can cause the mid-cycle sensation. The side may change from one cycle to the next, and the timing is not always identical for everyone.

A recurring pattern that stays mild and settles without treatment can fit ovulation pain. Pain that is new, persistent, worsening, or unrelated to the middle of your cycle can have another cause, such as endometriosis or an ovarian cyst, and deserves a clinical assessment.

What you can do

A warm bath or heating pad over the lower abdomen may relax tense muscles and reduce cramping. Rest, gentle movement, and drinking enough fluids can also make the episode easier to manage. Keep a simple record of the cycle day, side, duration, severity, bleeding, and other symptoms; this can help show whether the pattern matches ovulation.

If you can normally take them safely, an over-the-counter pain reliever may help. Follow the product directions, and check with a pharmacist or clinician first if you are pregnant, have kidney or stomach problems, take blood thinners, or have been advised to avoid a particular medicine.

When to seek care

Seek urgent medical help for sudden or severe pelvic pain, fainting, marked dizziness, heavy vaginal bleeding, fever, repeated vomiting, a rigid or very tender abdomen, or pain with a possible pregnancy. These symptoms can signal a problem that should not be attributed to ovulation without an examination.

Arrange a medical appointment if pain keeps returning with increasing intensity, lasts beyond your usual episode, interrupts daily activities, occurs with pain during sex or urination, or comes with unusual discharge or bleeding.
