Mittelschmerz and ovulation timing

Ovulation pain is usually felt in the lower abdomen or pelvis and may stay on one side. Some people notice a short-lived twinge; others feel an ache that lasts longer. The sensation may happen as the follicle stretches, releases an egg, or irritates nearby tissue.

Because cycles and symptoms vary, pain is not a clock for ovulation. Endometriosis or an ovarian cyst can produce pain that resembles mittelschmerz, especially when it is severe, prolonged, or accompanied by bleeding or digestive symptoms.

Write down the cycle day, side, start and end time, intensity, bleeding, discharge, and any urinary or bowel symptoms. Compare the pattern across cycles. Ovulation tests and cervical mucus observations can add information, while basal temperature usually confirms a pattern after ovulation rather than predicting it.

For a familiar mild episode, try warmth, rest, and a suitable non-prescription pain reliever. Ask a pharmacist or clinician about medicine choice if you may be pregnant or have a medical condition.

Obtain urgent assessment for sudden severe pain, fainting, fever, vomiting, heavy bleeding, or pain with a possible pregnancy. Seek a routine review if episodes are becoming stronger, lasting longer, or occurring outside your usual mid-cycle pattern.
