Mittelschmerz and getting pregnant

Mittelschmerz is mid-cycle pain that may be felt on one side of the lower abdomen. When it occurs near ovulation, sex around that part of the cycle may coincide with a fertile window. The symptom varies between people and from cycle to cycle, so it is only one clue.

Pain does not assess egg quality, sperm health, fallopian tubes, or the uterus. Endometriosis and ovarian cysts may also cause pelvic pain and can affect plans for pregnancy in different ways. Persistent or atypical pain should not be dismissed as ovulation pain.

Track cycle dates, cervical mucus changes, pain, and ovulation-test results if you use them. Having sex regularly through the possible fertile window may be more practical than waiting for pain, especially when cycles vary.

Take a pregnancy test if a period is late, following the test instructions. If conception is taking longer than expected or cycles are very irregular, arrange a fertility discussion rather than trying to interpret pain by itself.

Seek urgent care for intense or sudden pelvic pain, fainting, fever, vomiting, heavy bleeding, or pain when pregnancy is possible. Make a routine appointment for pain that lasts, repeatedly interrupts activities, occurs after sex, or appears at times unrelated to ovulation.
